The IRS has requested additional information be included with your tax filing this year.  This is due to the high volume of fraudulent returns that were filed in recent years, costing us taxpayers, as well as the US Treasury, millions of dollars.  In addition to the normal tax forms you will need to bring, you will also need to bring your current government-issued identification card - for most of us this means a valid Driver’s License.  This applies to both spouses with a Married Filing Joint tax return status.  

For taxpayers claiming the Earned Income Tax Credit or Child Tax Credit you will need to bring proof of residence of any dependent child(ren).  This can be a school or medical record, for example, showing the child’s name with the taxpayer’s address.  

If you are claiming a child on your return who does not live with you due to divorce or separation, you will need to provide a completed Form 8332 "Release of Claim to Exemption," which must be signed by the custodial parent.  This form is available on my "Resources" page.

Common tax forms you may receive / need …

  • W-2 - employees receive these from their employer
  • 1099-MISC - may be from independent contract work, rents or royalties received, or other
  • miscellaneous income
  • 1099-R - retirement account income - may be from IRA, 401(k), pension, annuity, etc
  • 1099-INT - from interest income
  • 1099-DIV - from dividend income - mutual funds, investment accounts
  • 1099-B - sales of stocks, bonds, other securities
  • 1099-SSA - from social security, SSI, SSDI, etc
  • K-1 - from ownership of company, s-corporation, partnership, energy company, etc
  • 5498 - statements of account balance, contributions to IRAs, etc
  • 1098-T - shows tuition amounts billed, paid, etc - beginning with tax year 2016 you must have receipts or some other "proof of payment"
  • showing amounts paid for tuition and books in addition to the 109-T must be provided
  • 1098 - mortgage interest, property taxes, mortgage insurance
